By signing this document, I expressly recognize my legal commitment to its terms, encompassing entities such as Sunshack I LLC, World Gym Pensacola, Salt Athletics, HaloTherapy Center LLC, and EASSZ LLC under the umbrella of "World Gym Fitness Center."

I am fully aware of potential risks associated with cardiovascular exercise, weightlifting, weight training, and bodybuilding, including equipment malfunctions and negligence-induced injuries. Voluntarily assuming these risks, I waive any liability claims against World Gym Licensing, Ltd., its affiliates, and representatives.

Affirming my fitness and obtaining medical clearance, I willingly accept all associated risks. I declare good health without conditions hindering my engagement in exercise activities.

The introduction of the Vitality Booth Wellness Center by HaloTherapy Center, LLC to World Gym Pensacola & Cantonment includes ionized water and therapies like halo therapy, red light therapy, oxygen-rich environments, and ozone generators. I recognize the importance of consulting a medical professional before partaking in these therapies.

Acknowledging World Gym Fitness Centers don't provide medical insurance, I release them from injury claims on their premises. I indemnify them against actions related to injuries, even if due to negligence.

I agree to adhere to World Gym Fitness Centers' rules; non-compliance may result in membership cancellation with a refund for any remaining period.

Despite the option to sign a new release per visit, I choose to forgo that and understand this release remains effective beyond the signing date. I commit to completing the fitness orientation within seven days of signing this membership agreement.

In confirming my understanding, I willingly intend to be legally bound by these terms, considering this document a comprehensive waiver and disclaimer for the benefit of World Gym Fitness Centers and World Gym Licensing, Ltd. My signature affirms my agreement hereto.
