Akron Member & PT Cancellation/Freeze Request Form

Member Cancellation request

There is a $19 cancellation processing fee. The account must be current in order to cancel with a 31-day written notice. All Cancellations require a 31-day written notice at World Gym, before the first of each month. The processing fee and any draft with in the 31-day notice must be collected at the time of cancellation.

Membership freeze request

All freezes must be received 10 days before next draft date. Maximum freeze is 3 months without a doctor’s note. Memberships can only be frozen 2 times per calendar year. The account must have a zero balance. Even if the account is on a freeze the bi-annual fee will still be charged.

Personal training Cancellation request

The account must be current in order to cancel with a 60-day written notice. All Cancellations require a 60-day written notice at World Gym, before the first of each month. The draft with in the 60-day notice must be collected at the time of cancellation.
